ACCOUNTANCY

Respondent: Mark Fontenot, Cookeville (Putnam County)

Violation: Misappropriated funds while serving as treasurer of a non-profit organization

Action: Suspension, civil penalty of $5,000 and two penalty hours of state specific ethics CPE

 

BOARD OF EXAMINERS FOR LAND SURVEYORS

Respondent: Ray Allen Maples Jr., Quebeck (White County)

Violation: Respondent violated rules of responsible charge of service.

Action: $1,000 civil penalty

 

BOARD OF FUNERAL DIRECTORS AND EMBALMERS

Respondent: Hall Funeral Home LLC, Celina (Clay County)

Violation: Aided or abetted an unlicensed person to practice within the funeral profession, failed to obtain and maintain the latest inspection report for the crematory utilized by the funeral home, and failed to provide a description of the merchandise purchased and the reason for embalming on contracts

Action: $750 civil penalty

 

BURIAL SERVICES

Respondent: Mt. View Cemetery LLC, McMinnville (Warren County)

Violation: Failed, within the time specified, to deposit moneys into the trust funds, file annual reports, and properly update the master map

Action: $1,000 civil penalty

 

COSMETOLOGY & BARBER EXAMINERS BOARD

Respondent: Suparery Pranee, Cookeville (Putnam County)

Violation: Unlicensed activity; sanitation violations

Action: $750 civil penalty

 

Respondent: Fashion Nails and Spa, Cookeville (Putnam County)

Violation: Sanitation violations

Action: $500 civil penalty

 

Respondent: Thanh Van Thi Phan, Cookeville (Putnam County)

Violation: Sanitation violations

Action: $500 civil penalty

 

Respondent: Rose’s Nail Spa, Cookeville (Putnam County)

Violation: Unlicensed activity; ID tag violations

Action: $2,250 civil penalty

 

REAL ESTATE COMMISSION

Respondent: Donnita S. Hill, Cookeville (Putnam County)

Violation: Failure to disburse escrow money in a proper manner

Action: $1,000 Civil Penalty, attend one regularly scheduled meeting of the Tennessee Real Estate Commission (in its entirety) within 180 days of Respondent’s execution of this order

Violation: Sanitation violations; ID tag violation

Action: $750 civil penalty

 

MOTOR VEHICLE COMMISSION

Respondent: Auto Credit Sales & Rentals, Baxter

Violation: Failure to maintain a temporary tag log and issuing more than two temporary tags

Action: $2,500 civil penalty
